Bipolarity in the Elderly

Abstract

|
Elderly bipolar disorder is much more common than previously thought. But considering the clinical impression of elderly with labile mood, cognitive decline and behavioral disturbances, bipolar disorders as a rule have been overlooked in the differential diagnosis. I intended to compare the similarities between bipolar disorder and dementia of Alzheimer¡¯s disease-the prototype of cognitive dysfunction-with respect to clinical features, premorbid temperament, neurobiology, response to treatment, and common pathophysiology, such as cerebrovascular lesion, as well as extending the concept of bipolar spectrum disorders. Both disorders share many features, however it is unclear whether they are entirely separate disorders, merely share common risk factors, or bipolar disorder is a causal factor of dementia. Because cognitive and mood symptoms in the elderly might in fact be a manifestation of late-onset bipolar spectrum disorders, we need broader considerations when diagnosing and treating them.
